Brow
lift surgery is a great way to rejuvenate the upper
1/3 of the face. In fact, many patients seeking eyelid
surgery would benefit more from a brow lift than just
"doing their eyes." The aging brow takes on
a "tired" and sometimes "angry"
look. Furrows between the eyes appear, transverse forehead
lines develop, and the eyebrows descend. A properly
performed brow lift can treat these complaints without
creating a "startled-look." Dr. Wallach uses
the most modern techniques and for many patients he
can perform this using three to five small incisions
behind the hairline using a small television camera
called an endoscope. This eliminates the more traditional
incision that extends from ear to ear across the scalp.
The surgical procedure usually takes between 1-2 hours
with a recuperation time of 7-10 days.

Eyelid
surgery is a great "eye-opener" for many patients
and should not be confused with the rejuvenation performed
with a brow lift (see Brow Lift Surgery section). The
youthful, natural looking eye is essential to provide
harmony with the rest of the face; it is often the first
area requested to be treated. It can be done in conjunction
with other face rejuvenating procedures. Dr. Wallach
recreates a natural looking youthful eye by removing
only necessary fat and skin from the upper and lower
lids. He tries to conserve as much of the soft tissue
in and around the eye to prevent the "hollowed-eye"
look which is a sign of aging. Dr. Wallach will often
treat the lower eyelids using an incision on the inside
of the lid. The surgical procedure usually takes between
1-2 hours to perform with a recuperation time of about
7-10 days.

No
one patient undergoes the same exact facelift. It is
very important when performing the surgery to avoid
creating "telltale" signs, or unfavorable
looking results that make the face appear "unnatural."
This would include distortion of the ear shape, hairline
and/or sideburn. Dr. Wallach usually works on the deeper
layers of the face to provide the so-called "lift"
and create a "natural" result. The deeper
tissues of the face are mobilized to recreate the soft
tissue appearance of a youthful face. Most importantly,
he does not "pull' on the skin which can result
in a distortion of the face. He uses the analogy of
a bed to describe this. The deeper tissue of the face
is like the mattress of a bed and the skin is like a
sheet covering the mattress. In order to smooth out
the bumps in the mattress, or improve the contour of
the face you have to work on the mattress, or the deeper
tissues of the face. Dr. Wallach always tries to provide
the maximum result with the minimum amount of incisions.
Therefore, some patients may undergo a short- scar facelift
(mini-facelift) to improve their neck and jowls while
others may require a more involved procedure that improves
the neck, jowls, and cheeks as well. The surgical procedure
usually takes between 2 and 4 hours to perform. The
recuperation is from 1-2 weeks. Dr. Wallach has presented
at national meetings and published articles on this
subject in peer-reviewed journals.

Corrective
ear surgery whether it is for prominent ears or for one
of several ear deformities is a relatively straightforward
procedure that can change the size and shape of the ear.
This is most commonly performed on young children during
early school years but it is not uncommon to perform it
on older patients. Incisions can be placed either behind
the ear or in a crease within the ear. The ear cartilage
can then be treated by removing a portion of it or folding
it on itself and securing it in position with sutures.
The surgical procedure usually takes 2-3 hours to perform
and the recuperation takes about 1 week. The actual techniques
applied are dependent upon the nature of the deformity.
Dr. Wallach has published an article on this subject in
a peer-reviewed journal.

Nasal
surgery has evolved over the years from a "cookie
cutter, one-type fits all" procedure to now individualizing
the surgery to provide a natural result while also maintaining
or improving breathing for most patients. This may mean
removing a bump on the nose or refining the tip shape
in some patients. There are some patients that have
a "flat and wide" nose that may need narrowing of the
nose or even raising of the profile of the nose. The
surgical procedure usually takes between 1-2 hours with
a recuperation of 1-2 weeks. Dr. Wallach has presented
at national meetings and published papers on rhinoplasty
in peer-reviewed journals.
