In general, many men have mostly a fatty component to the chest when it is enlarged. This is determined on physical exam in the office. If it’s felt to be fatty in nature then liposuction can be the adequate treatment. When there is also glandular tissue that does not respond to liposuction, then I will combine this technique with an excision or direct approach which is usually done along the lower border of the areola. If there is also loose chest skin, I might need to remove excess skin as well.
