I will always choose the least invasive, least amount of incision procedure to produce the best results. That being said, I will not compromise the result for the former or for the latter. For a patient that needs facial rejuvenation due to soft tissue descent but does not have alot of loose skin or loose skin mainly confined to the upper one-third of the face and jowls, I may offer a limited scar facelift or a mini-facelift to accomplish the goals necessary to achieve excellent results. This may include a SMASectomy or SMAS Plication or even a High SMAS elevation. If the patient has too much loose skin and needs more redraping then a full incision facelift may be necessary combined with a neck lift procedure. No two patients are exactly the same, no two patients have the exact same anatomy, and this is why each patient may be have a different surgical plan.
